The wash available in South Africa is Benzac AC 5 Wash, containing 5% benzoyl peroxide. For simplicity, I will refer to it as Benzac Wash throughout this article. (Benzac)
Both contain benzoyl peroxide, but the way they are used is different.
Benzac Wash is a wash-off treatment that is applied during cleansing and rinsed away. Benzac AC 5 Gel is a leave-on treatment that remains on the skin after application.
That difference can influence which product is more practical for your acne, where you are treating it and how well your skin tolerates benzoyl peroxide.

When Might Benzac AC 5 Gel Be a Better Choice?
Benzac AC 5 Gel may be particularly useful when inflammatory acne involves an area where applying a thin layer of leave-on treatment is practical.
For example, someone repeatedly developing inflammatory pimples across the cheeks, forehead or jawline may be advised to apply benzoyl peroxide to the acne-prone area rather than treating only individual visible pimples.
A leave-on formulation provides prolonged contact with the skin.
However, more contact does not mean you should apply more product. A thin layer is sufficient, and excessive application can increase dryness and irritation.

When Might Benzac Wash Be a Better Choice?
Benzac Wash can be particularly practical when acne affects larger areas of skin.
Body acne commonly involves the:
- Chest
- Shoulders
- Upper back
Applying leave-on gel across a large area every day can be inconvenient. Incorporating a benzoyl peroxide wash into showering may therefore be easier for some people.
According to the South African product instructions, Benzac AC 5 Wash is gently applied to the affected area, left on the skin for 30 seconds and then rinsed off. A wash-off formulation may also suit someone who prefers not to leave benzoyl peroxide on the skin for prolonged periods.
However, body acne can become severe and can cause permanent scarring. A benzoyl peroxide wash should not delay medical treatment if acne is widespread, deep or scarring.

Either formulation may potentially have a role, depending on the acne and the individual’s skin.
A leave-on gel may be more practical when treating a defined acne-prone facial area.
A wash may appeal to someone who prefers a rinse-off product or whose treatment plan specifically incorporates a benzoyl peroxide cleanser.
The decision should not be based simply on which product seems stronger.

Benzac Wash vs Benzac AC 5 Gel: Which Is Better for Sensitive Skin?
There isn’t one answer for every person with sensitive skin.
Because Benzac Wash is rinsed away after a short contact time, some people may prefer a wash-off formulation. However, this does not automatically mean it will be less irritating, and Benzac Wash can still cause dryness or peeling.
Similarly, Benzac AC 5 Gel can sometimes be used successfully in sensitive skin when introduced gradually and combined with appropriate moisturisation.

Persistent burning, marked redness, swelling or blistering should not simply be accepted as normal treatment irritation.
Can You Use Benzac Wash and Benzac AC 5 Gel Together?
Potentially, but using both does not automatically mean better acne control.
Because both products contain benzoyl peroxide, using them on the same area can increase dryness, peeling and irritation, particularly when treatment is first introduced.
There are, however, situations where more than one benzoyl peroxide-containing product may appropriately form part of an acne treatment plan.
For example, a dermatologist may recommend Benzac AC 5 Wash for acne affecting the chest or back, while a leave-on benzoyl peroxide treatment is used as part of the facial acne regimen.
If both products are being used, they do not necessarily need to be used in the same routine. They may be separated between morning and evening:
Morning:
Benzac AC 5 Wash → Moisturiser → Sunscreen
Evening:
Gentle cleanser → Benzac AC 5 Gel → Moisturiser
If your skin is sensitive or you are new to benzoyl peroxide, treatment may also be introduced gradually or the frequency reduced initially, depending on your treatment plan and how your skin responds.
For example, rather than using both benzoyl peroxide products every day from the beginning, they may be used on different days initially and increased according to tolerance when clinically appropriate.
This approach is consistent with the broader principle of gradually introducing potentially irritating topical acne treatments. NICE specifically recommends considering alternate-day or short-contact application initially for topical treatments such as benzoyl peroxide or retinoids when reducing irritation is necessary. The same principle can apply when Benzac AC 5 Wash is prescribed alongside another benzoyl peroxide-containing acne treatment, such as Epiduo (adapalene + benzoyl peroxide). The treatments may be separated by treatment area, time of day or frequency to improve tolerability.
The important point is that using two benzoyl peroxide-containing products should have a clear clinical purpose. More benzoyl peroxide is not automatically better, and the regimen should be adjusted if significant irritation develops.
What If Neither Is Controlling Your Acne?
Benzoyl peroxide is particularly useful for inflammatory acne, but it does not target every acne pathway equally.
If you have significant blackheads and whiteheads, a topical retinoid may also be needed. Current acne guidelines support topical retinoids and benzoyl peroxide as important acne therapies and recommend combining treatments with different mechanisms of action when appropriate. Deep nodules, cyst-like lesions, widespread acne or acne that is beginning to scar may require prescription treatment.
The answer to persistent acne is therefore not always:
Wash → Gel → more benzoyl peroxide
Sometimes the treatment plan itself needs to change.

When choosing Benzac Wash vs Benzac AC 5 Gel, I would not decide purely according to which product sounds stronger.
I would first look at what type of acne we are treating, where it is occurring, how extensive it is and how well the patient’s skin tolerates benzoyl peroxide.
For facial inflammatory acne, a leave-on gel may be very practical. For acne spread across the back or chest, a wash can make treatment of a larger area much easier.
Neither formulation needs to do everything.
If acne is persistent, deep or beginning to scar, it is more important to reassess the overall acne treatment plan than to keep adding benzoyl peroxide products.
